1. Declare Self-Employment Status
Provide a clear written statement confirming your status as self-employed. This declaration affirms your independent working arrangement when responding to a practice information notice under s132(4)(a).
2. List Practice Addresses
Disclose the address of every premises where you practise. This includes primary clinics, secondary locations, and any addresses used for providing remote services (e.g., telehealth). Patient residential addresses for home visits are excluded.
3. Include Business Names
Document any business name(s) under which you operate professionally. If operating through a practice company, disclose the company name(s) used for providing services. This aids in identifying your professional branding and operational structure.
4. Detail Co-Practitioners
List the full names of all other registered health practitioners with whom you share premises and associated cost arrangements. This applies even if co-practitioners operate through separate entities. Check service entity lists or contact administrators if needed.
5. Financial Details Not Required
You are not required to disclose the specific financial details or mechanics of your cost-sharing agreements (e.g., percentage splits, service fees) as part of this declaration under Section 132.
